Page 2 During the investigation on 5/17/2023, LPA interviewed staff present, obtained the facility roster, reviewed incident reports, and reviewed Face to Name Transition Sheets. On the Face to Name Transition Sheets, next to the child\342\200\231s name, there is a section listed as \342\200\234Health Checks.\342\200\235 On 5/17/2023 four staff were interviewed. All four staff interviewed indicated they conduct health checks and look for various symptoms. All staff mentioned, if a child is not feeling well and must be sent home, the child is isolated from other children in care until the parents arrive. Parent interviews were conducted via phone call on 5/18/2023. Five parents were able to be interviewed out of 11 parents contacted. Parents were asked the following questions: How do you feel about the care and supervision your child receives? Have you ever been notified of your child being ill while present at the facility? Have you ever witnessed staff conducting health checks? Four out of 5 parents interviewed indicated they were satisfied with the care given by the facility staff and had no concerns to report. Two parents indicated they were notified by staff when their child was ill and was asked for the child to be picked up. One parent mentioned they have observed a staff conducting temperature checks. Based LPA observations, Face to Name Transition Sheets which shows Health Checks are being conducted at the facility, interviews with staff and parents, there was not enough evidence to substantiate the allegation. The Orange County Child Care Office has investigated the complaint alleging that the (1) Staff are allowing ill children into daycare: although the allegations may have happened or is valid, there is not enough preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit. Exit interview conducted and report was reviewed with Director America Lopez. Notice of Site Visit was provided to America Lopez and posted on the door. The notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The licensee was provided a copy of their appeal right (LIC 9058 1/16) and their signature on this form acknowledges receipt of these rights.
