What the inspector wrote

Page 1 of 1 Licensing Program Analyst (LPA), Mahnaz (Nancy) Malek conducted an onsite case management inspection for the purpose of amending a report that was issued earlier today dated 1/9/2025. There were a total of 11 infant and toddlers with 5 staff in two different rooms. There were 3 infants with one staff in Room G, and 8 toddlers with 4 staff on the playground. A review of the Facility Personnel Report Summary on this date indicates all facility who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The amended report was provided to director, Thanice Epps. Notice of Site Visit was posted. The notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The director, Thanice Epps was provided a copy of their appeal rights (LIC 9058 1/16) and their signature on this form acknowledges receipt of these rights. Exit interview was conducted with director, Thanice Epps.

    Responsibility for Providing Care and Supervision for Infants- (a) In addition to Section 101229, the following shall apply:(1)Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement was not met as evidenced by a self report incident, 3 staffs' interviews, and reviewing documents that S1 left two 10 months old children unattended in the room for approximately 3 to 4 minutes in order to get a buggy out of the storage room away from the room during transitioning children to playground. This is an immediate risk to the health and safety of children in care.
