Based on the record reviewing today, the licensee did not comply with the section cited above in 4 counts out of 8 files that staff # 3, 4, 5, and 7 did not have the required immunization such as MMR and Pertussis in their files which poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants- (a) In addition to Section 101229, the following shall apply:(1)Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement was not met as evidenced by a self report incident, 3 staffs' interviews, and reviewing documents that S1 left two 10 months old children unattended in the room for approximately 3 to 4 minutes in order to get a buggy out of the storage room away from the room during transitioning children to playground. This is an immediate risk to the health and safety of children in care.
