What the inspector wrote

An Informal Meeting was conducted on this day in the Orange Regional Office. Present during the meeting were Nguyen Nick Tran, Licensing Program Manager (LPM), Cindy Nguyen & Vivian Trinh, Licensing Program Analysts (LPAs), DeAnne McCashland, District Manager and Irene Guereca, Director. The purpose of this informal meeting is to discuss the followings : 101229 (a)(1) Responsibility for Providing Care and Supervision (05/15/2024) 101229(a)(1) Responsibility for Providing Care and Supervision. Child was found by the curb near the drop off and pick up zone. LPA measures an approximate 151ft from the playground office door to where the child was found. C1 left the playground office door was at 4:29pm and C1 returned to the office by the witness at 4:32pm. (08/24/2021) 101229(a)(1) Responsibility for Providing Care and Supervision. Based on interviews and statements provided, the facility staff failed to supervise the two-year-old child on the playground whereby the child was able to slip away out of the teacher\342\200\231s sight. 101238(a) Buildings and Grounds ( 11/18/21) Based on interviews and record review on 08/17/2021, C1 went through unsecured fence; got out of the facility for approximately 5-10 mins; facility failed to keep the fence in good repair this led to the incident. During the informal conference, District Manager and Director was given the opportunity to discuss violations and actions taken to address the deficiencies as follow. 101229 (a)(1) Responsibility for Providing Care and Supervision Facility continued to send out reminders to parent regarding to make sure parents only pick up your own child(ren). Facility also added the door codes/latches and face-to-name supervision procedure. Continued on Page LIC 809C Page 809C Director conducted a safety training for outdoor play and safety with all staff. During busy drop pick up, someone will be in the office to provide additional checks and supervision during dismissal. 101238(a) Buildings and Grounds A technician was out the next morning to check on the fence, closed the gap and repaired the fence. We also put in place a daily physical plant checklist, if the fence can be open, there is an alarm. The director indicated that all the safety measures have been taken including repairing, training, and reviewing the safety policy with all the staff in school to prevent any future incident. The following was discussed and provided with the District Manager & the Director: The licensee was advised of the responsibility to know & understand Title 22 Regulations. The facility must always remain in compliance with Title 22 regulations. The licensee was advised to check the Childcare Licensing web site at www.ccld.ca.gov for quarterly updates, training, forms, and regulations. Childcare Licensing E-Learning Modules. TSP and Supervision at a Glance. Exit interview was conducted with DeAnne McCashland, District Manager and Irene Guereca, Director, who agrees with the above.
