(1) No child shall be left without the supervision of a teacher at any time, Supervision shall include visual observation. This requirement is not met as evidenced by: Based on the information gathered from the interviews conducted and statements received, it was determined that C1 was left alone on the playground unsupervised. This is an immediate risk to the health and safety of the children in care.

C1 was able exit the facility, out the front entrance door to the curb near the drop off and pick up zone an approximate 151 ft. This poses an immediate Health and Safety risk to the children in care.

This poses an immediate risk to the health and safety of the children in care.
