What the inspector wrote

On 12/22/25, at 9:30 am, a case management inspection was conducted today by Licensing Program Analyst (LPA) Patricia Duron who met with the Facility Representatives, Gabriela Preciado and Irene Guereca and informed them of the purpose of the visit. Facility Representative Gabriela Preciado lead LPA on a walk through the facility. Census was taken. Census was 36 preschool children with 6 staff. A self-reported incident by the facility was received at the Regional Office on 11/25/25 which stated that on 11/24/25, Child #1 (C1) was left on the playground when Staff #1 (S1) transitioned the class to the classroom. Staff #2 (S2) saw C1 on the playground and brought C1 to the Director. A review of criminal record clearances indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Based on a self-reported incident, S2 witnessed C1 left alone on the playground unsupervised. Based on staff statements received by the department about the incident, S1 was not aware that C1 was left on the playground unsupervised. S2 stated they walked outside and witnessed C1 in the corner of the playground. S2 walked over to C1 to see if they were ok. S2 walked C1 to the Facility Representative, Irene Guereca and informed Irene Guereca of the situation. Facility Representative, Irene Guereca stated after the incident the facility provided training to all staff members reviewing the procedure for face to name transition sheet plus discussing the importance of supervision and safety to children in care. Based on the information gathered from the interviews conducted and statements received, it was determined that C1 was left alone on the playground unsupervised. This is an immediate risk to the health and safety of the children in care. LPA Patricia Duron informed facility representative, Irene Guereca that this report dated 12/22/25 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. LPA Patricia Duron informed the facility representative to provide a copy of this licensing report dated 12/22/25 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Irene Guereca. Page 2 of 2. End of Report.
