What the inspector wrote

(page 2) From the information gathered by LPA at the time of this inspection, the allegation of Staff left day care child unattended has been substantiated. Based on this gathered information, the preponderance of evidence standard has been met, and therefore the above allegation has been found to be substantiated. Due to the Type A violations cited today, the facility shall post, and provide copies, of the report to parents/guardians of the children in care at the facility by the next business day, and shall provide to the parents/guardians of children newly enrolled at the facility during the next 12 months. The facility is to keep Acknowledgement Receipt (LIC 9224) signed by parents in each child\342\200\231s file. In addition, the licensee shall immediately post upon receipt the Proof of Correction for 30 consecutive days. Appeal Rights were explained and the facility was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. A Notice of Site Visit form was given to licensee and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. The above was not met as evidenced by:Based on interviews with staff, LPA determined that a child #1 was left by staff unsupervised in the facility parking lot, for approximately 2 minutes. This poses an immediate threat to the child/children's health, safety, and personal rights.
