What the inspector wrote

On 5/12/2026, Licensing Program Analyst (LPA) M. Do and Licensing Program Manager (LPM) M. Malane conducted an unannounced onsite inspection for the purpose of an Annual Required Inspection. LPA Do and LPM Malane were led on a tour by Director Begole. There were 30 children and 5 staff present, including the Director. Facility hours are 6:30 am to 6:00 pm Monday to Friday. During today’s inspection of the indoor activity space, LPA Do observed that detergents, cleaning compounds, medicines, and other items which could pose a danger if readily available to children were stored inaccessible to children. Poisons / hazardous items were not kept on the premises. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. There was drinking water available to children indoors and outdoors via water bottles with the child’s name on it. The children’s restrooms were clean and sanitary. Children nap on cots. Bedding was stored separately and staff stated that parents take the bedding home to be washed weekly. The facility has conducted an emergency drill within the past six months on 12/4/25. The facility has a working carbon monoxide detector. Facility met all posting requirements. The California Child Passenger Safety law was posted by the entrance of the facility. Telephone number of the local health department was also posted at the entrance. Continue to Page 2 Page 2 The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four (4) feet in height. The surface of the outdoor activity was free of hazards. The cushioning material rubber mats commercially produced for the purpose around and under the climbing equipment, slides and other similar equipment appeared to be enough to absorb falls. Playground that was located outside of Classroom 2 was observed to have debris, propped up tables, broken furniture stacked in an unsafe manner for children, door threshold had exposed nails, and sharp, rusty screws were stored in an open container accessible to children (See LIC 809D for deficiency cited). LPA observed a pool at the facility. Entrance to the pool is through the playground. There was a gate that led to the pool. The swimming pool was equipped with a fence enclosure which measured five (5) feet in height. The gate swings away from the pool is self-closing and self-latching. There was a body hook and a 17” life ring approved by the United States Coast Guard. Director stated that the pool was not being used by daycare children. However, daily inspection was not completed (See LIC 809D for deficiency cited). Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). Based on interview and record review, the licensee did not comply with the section cited above in that they did not test for lead contamination levels which poses a potential health, safety or personal rights risk to persons in care (See LIC809D for deficiency cited). Director stated they will contact the owner and contact LPA by 05/25/2026. Continue to Page 3 Page 3 Safe Sleep: LPA Do discussed the safe sleep regulations with the Director. However, sleep logs were not completed for napping infants (See LIC809D for deficiency cited). LPA also informed Licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Staff Records: A review of the Facility Personnel Report Summary on this date 05/12/26 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Continue to Page 4 Page 4 Five (5) staff files were reviewed for staff present during the facility inspection on this date. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1596.7995 states, a person shall not be employed or volunteer at a childcare center if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for four staff were reviewed. However, 1 out of 5 staff did not have a health screening (See LIC809D for deficiency cited). Beginning March 31, 2018, H&S Code 1596.8662 requires all employees to complete mandated reporting training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 11/15/2026. Children Records: Children's records were reviewed, and there was a separate, complete and current record for each child. A random sample of five (5) of children's files were reviewed for documentation of the child’s name, address, and telephone number of the child’s authorized representative and of relatives or others that can assume responsibility for the child if the authorized representative cannot be reached, when necessary, Immunization records, Consent for Emergency Medical Treatment, Notification of Parent’s Rights (LIC995A), medical assessment and found to be in compliance. However, 1 out of 5 children did not have a physician’s record (See LIC809D for deficiency cited). Sign in/Sign out procedure was reviewed and meets regulation requirements. This facility utilizes an electronic sign in/out system. Food Service: Snacks were provided. Food prep areas were clean and sanitary. Food was properly stored. Menus were posted where they could be reviewed by parents. In the areas that were evaluated, Type B deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.
