On January 8, 2026, at 6:20pm, a case management inspection conducted on this by Licensing Program Analyst (LPA) Long Pham to issue an amended LIC9099 report, update the LIC 9099, LIC9099C pages dated 12/16/2025, and obtain signatures. LPA met with Director Jasmin Canales and toured the facility. LPA observed 2 staff and 0 infant. A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the visit, it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Please see "Amended" LIC 9099 report dated 12/16/2025 and updated LIC 9099C page for correction. No deficiency was observed during today’s inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative Jasmin Canales. Appeal Rights were explained. The Facility Representative was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. End of Report.
