What the inspector wrote

On 6/30/2025, Licensing Program Analyst (LPA) A. Silva conducted a Case Management – Deficiencies to address deficiencies observed during the visit. Upon arrival, the LPA met with director Mojgan Vasseghi. An on-site Facility Personnel Report Summary review indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Census was 14 infants. The facility was operating within ratios and capacity. During today’s visit, the LPA requested a sing in/out sheet for infants C6, C7, C8, and C9 for June 10, 2024. The director provided a sing in/out sheet that did not include the full legal signature of the person who brings the child to, and removes the child from, the center. In addition, the times shown on the sign in/out sheet did not reflect the actual times the children were present at the facility. For example, the LPA observed children C6, C7, C8, and C9 in the facility at around 9:00 a.m. on June 10, 25. The sign in/out sheet stated the children were in at 4:29 p.m. The Children Supervision Record reflected different times for all for children, and the times were before 9:00 a.m. for all four children on 6/10/24. The LPA inquired about signatures and times. The director stated the signature does not show and the times reflected are the times when the children were sing in into a different room. The director attempted to provide a different sign in/out sheet. However, the second sheet did not show the name of C9 and instead showed the name of C9’s sibling. The sheet did not clearly identify the sign in/out times or the action. Instead, the colum for times was labled “Action Time.” The Action Time in one page showed 7:29 a.m, 10:27 a.m., and 4:40 p.m. Another page showed attendance date was 6/10/25 and in the Action Time colum the date was 6/16/25 and the time was 3:29 p.m. The pages that contained the signatures, which the director stated were associated with the June attendance sheet, was dated 9/15/2021. The LPA discussed the CCR Section 101229.1 Sign In and Sign Out (a) In addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following: (1) The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. (b) The person who brings the child to, and removes the child from, the center shall sign the child in/out. 101229.1 Sign In and Sign Out (a) In addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following: (1) The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. (b) The person who brings the child to, and removes the child from, the center shall sign the child in/out. The director stated there were no other sing in and sign out sheet was provided during the visit. Violations of the CCR, Title 22, Section 101229.1 Sign In and Sign Out were observed and cited today. An exit interview was conducted and appeal rights and a notice of site visit were provided to the licensee. End.

    101212(d) Reporting Requirements, Upon the occurrence, during the operation of the child care center, of any of the events specified in (d)(1), a report shall be made to the Department. This requirement is not met as evidenced by: Based on interviews and records review, the licensee did not meet the requirement to report an unusual incident involving a child who needed emergency medical treatment and was hospitalized for 2 days or more. S1, S4, and S6 disclosed that the incident had happened. S6 stated the incident was not reported to licensing. This poses a potential risk to the health and safety of children in care.

    101226.1 Daily Inspection of Illness (a) The licensee shall be responsible for ensuring that children with obvious symptoms of illness … are not accepted. This requirement is not met as evidenced by: Based on interviews, records review, and observations, the licensee did not meet the requirement to not accept children with symptoms of illness. In addition, the licensee failed to notify the child’s authorized representative as required by 101226(a). The LPA observed children with obvious symptoms and S6 said the parents had not ben notified. This poses an immediate risk to the health and safety of children in care.
