Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

REPORTING REQUIREMENTS
101212(d) Reporting Requirements, Upon the occurrence, during the operation of the child care center, of any of the events specified in (d)(1), a report shall be made to the Department. This requirement is not met as evidenced by: Based on interviews and records review, the licensee did not meet the requirement to report an unusual incident involving a child who needed emergency medical treatment and was hospitalized for 2 days or more. S1, S4, and S6 disclosed that the incident had happened. S6 stated the incident was not reported to licensing. This poses a potential risk to the health and safety of children in care.

101226.1 Daily Inspection of Illness (a) The licensee shall be responsible for ensuring that children with obvious symptoms of illness … are not accepted. This requirement is not met as evidenced by: Based on interviews, records review, and observations, the licensee did not meet the requirement to not accept children with symptoms of illness. In addition, the licensee failed to notify the child’s authorized representative as required by 101226(a). The LPA observed children with obvious symptoms and S6 said the parents had not ben notified. This poses an immediate risk to the health and safety of children in care.

101229.1 Sign In and Sign Out (a) The licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following: ...This requirement was not met as evidenced by: Based on records review and interviews with the director, the facility does not have a sign in and sign out sheet that meets the requirement.The sing in/out sheets observed and obtained from the director were incomplete and/ or incorrect.
