    101212(d) Reporting Requirements, Upon the occurrence, during the operation of the child care center, of any of the events specified in (d)(1), a report shall be made to the Department. This requirement is not met as evidenced by: Based on interviews and records review, the licensee did not meet the requirement to report an unusual incident involving a child who needed emergency medical treatment and was hospitalized for 2 days or more. S1, S4, and S6 disclosed that the incident had happened. S6 stated the incident was not reported to licensing. This poses a potential risk to the health and safety of children in care.

    101226.1 Daily Inspection of Illness (a) The licensee shall be responsible for ensuring that children with obvious symptoms of illness … are not accepted. This requirement is not met as evidenced by: Based on interviews, records review, and observations, the licensee did not meet the requirement to not accept children with symptoms of illness. In addition, the licensee failed to notify the child’s authorized representative as required by 101226(a). The LPA observed children with obvious symptoms and S6 said the parents had not ben notified. This poses an immediate risk to the health and safety of children in care.

    Based on records review, the licensee did not comply with the section cited above in 3 of 5 staff, which poses/posed a potential health, safety or personal rights risk to persons in care. Two staff did not have a LIC503 or qeuivalent. One staff who had a complete an LIC503 did not have proof of tuberculosis testing.

  • ARTICLE 2. Administration of Child Day Care Licensing

    Based on records review, the licensee did not comply with the section cited above in 4 of 5 staff, which poses/posed a potential health, safety or personal rights risk to persons in care. LPA checked 5 staff files for compliance. Four of the staff did not have a current Mandated Reporter certificate. The director stated they would complete a new one soon.

    101428 Infant Care Personal Services(b) The infant shall be kept clean and dry at all times.(2) Each infant's clothing and diapers shall be changed as often as necessary to ensure that the infant is clean and dry at all times.This requirement was not met as evidence by records review and statements. On 06/09/2022 records reviewed revealed that the child in question was not provided a diaper change for a long period of time. Statement received by a staff stated that the children are not provided a diaper change when napping so the child is not interrupted. This poses a risk.
