What the inspector wrote

On 11/18/2025 , Licensing Program Analyst (LPA), K. Trinh conducted an unannounced Case Management - Plan of Correction inspection for the purpose of following up on a deficiency incurred by the facility on 10/28/2025. Upon arrival, LPA met with Licensee, Marsi Decasas, and was led on a tour of the facility. There were three (3) children and two (2) assistants present. During the tour, it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff, or other individuals who require caregiver background checks have received or are in the process of obtaining criminal record and child abuse index clearances or exemptions. On 10/28/2025, licensee was cited for one assistant, Staff 2 (S2), who did not obtain criminal record clearances prior to working in the home. Licensee stated that they would ensure S2 would obtain clearance prior to returning to the home. During today's visit, LPA was able to confirm that S2 was not present at the facility. Licensee provided a declaration (LIC855) stating that S2 left, and is no longer employed as a teacher at the facility since 10/28/2025. The following were discussed: Individuals who are 18 years of age or older living or working in the home must be fingerprinted cleared prior to being present in the facility. Adults must contact a LiveScan office complete LIC9163. If adult is fingerprinted cleared and associated to another facility, licensee must complete a Criminal Record Clearances or Exemption Transfer Request form. (Continue to page 2) (Page 2) In the areas that were evaluated, the facility was in compliance of the California Code of Regulations, Title 22, Division 12. The licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. Exit interview was conducted. The Notice of Site Visit was posted. Facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100.
