On 11/18/2025, Licensing Program Analyst (LPA), K. Trinh conducted an unannounced Case Management - Other inspection. Upon arrival, LPA met with Licensee, Marsi Decasas, and was led on a tour of the facility. There were three (3) children and two (2) assistants present. During the tour, it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff, or other individuals who require caregiver background checks have received or are in the process of obtaining criminal record and child abuse index clearances or exemptions. During today's inspection, LPA observed that Staff 2 (S2) did not have a copy of their immunization record or a mandated reporter training certificate in file; see LIC 809D for deficiencies. LPA provided the Licensee with a copy of the Health and Safety Code 1597.622 and 1596.8662. Violations of Health and Safety Code (HSC) Section 1597.622(c) and Section 1596.8662(b)(1) were previously cited on 10/28/2025. During today\342\200\231s visit on 11/18/2025, the facility is being cited in the same section of HSC. This is a repeated violation within 12 months, therefore, a civil penalty for repeated violations was issued. Exit interview was conducted. The licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. The Notice of Site Visit was posted. Facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100. End of report.
