PERSONNEL REQUIREMENTS

Based on observation and record review, the licensee did not comply with the section cited above in one count out of one file, Adult # 1 missing the current CPR and 1st aid which poses a potential health, and safety risk to persons in care. Adult # 1's Pediatric CPR/First Aid certifications expired on 9/17/2024 and the licensee has enrolled in part of the CPR and 1st aid training on 5/12/2025 and has not obtained a current/final certificate yet.

Alterations to Existing Buildings or GroundsPrior to making alterations or additions to a family child care home or grounds, the licensee shall notify the Department of the proposed changed, including, but not limited to, the following: Room additions to the family child care home. This requirement has not met as evidenced by licensee has moved from children's area from the master bedroom to living room only. Licensee has eliminated back door and side door use as well and not reporting the changes. This is a potential risk to the health and safety of children in care.

OPERATION OF A FAMILY CHILD CARE HOME

An emergency information card shall be maintained for each child and shall include the child's full name, telephone number and location of a parent or other responsible adult to be contacted in an emergency,.......... This requirement was not met as evidenced by the licensee's statement that she moved from children's area to living room and could not find the children's files. 3 out of 3 children did not have the files available. This is a potential risk to the health and safety of children in care.

ARTICLE 2. Administration of Child Day Care Licensing

Current Roster of Children Provided Care in Facility Roster- Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This requirement was not met as evidenced by licensee's statement that she has moved from the children's room to living room and could not find the roster. This is a potential risk to the health and safety of children in care.

(d) Disaster drills shall be conducted at least every six months. (2) The drills shall be documented. This documentation shall be kept in the child care center for at least one year. This requirement is not met as evidenced by licensee's statement that she has moved from children's area to living room and does not have access to the fire drill log. This is a potential risk to the health and safety of children in care.

PERSONNEL REQUIREMENTS

Based on observation, interview, and record review)], the licensee did not comply with the section cited above. LPA observed licensee First Aid/ CPR certification expired on March 7, 2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

OPERATION OF A FAMILY CHILD CARE HOME

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file review, licensee was only able to provide 2 out of 5 children files to LPA which poses/posed a potential health, safety or personal rights risk to persons in care.

INFANT SAFE SLEEP

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee was unable to provide LPA proof that she is documenting infant safe sleep. Licensee stated she did not have a safe sleep log.in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.
