What the inspector wrote

On September 23, 2025, Licensing Program Analyst (LPA) Vivian Trinh and (LPA) Long Pham conducted an unannounced capacity increase inspection from a Small Family Child Care home to a Large Family Child Care home. Fire clearance is approved on 9/16/2025. The LPAs met with the licensee, Bhorapkar, Anita, who provided a tour of the home. At 9:00AM, LPAs observed licensee caring for total Zero (0) day care children. The licensee was operating within the licensed capacity as specified on license. A review of the Facility Personnel Report Summary on this date 9/23/2025 indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Currently, there are 3 adults, including the licensee and three (3) minor child, living in the facility. Facility Day care hours are 8am-5pm, Monday through Friday. During today\342\200\231s inspection, LPAs and the licensee toured the inside and outside areas of the facility, sketch LIC 999 as accessible to childcare children. This is a two-story home with 3 bedrooms, 3 bathrooms, a living room, a family room, a kitchen, a laundry room, storage, closets, a front yard (not fenced), a backyard (fenced), and a garage. Licensee has placed a safety gate at the bottom of the stairs leading to the 2 nd floor, ensuring it is inaccessible to children who are in care. Off-limits areas are made inaccessible by means of baby gates, doorknobs, or latches. Licensee stated that OFF-LIMIT areas include: All bedrooms on the 2nd floor, the hallway, garage, some parts of the backyard, the jacuzzi and a fishpond, side yard, storage, and the backyard shed. The licensee acknowledged that children may never enter these off-limit areas. Continued Page 2. Page 2. I NDOOR INSPECTION: The childcare area consists of the living room, which is accessed through the front door. During the tour of the living room LPAs observed 2 aquariums that were surrounded by toy boxes and a changing table in the corner of the living room. The children's primary area for care is in the living room (Child care area), Room #2, and some parts of the backyard. LPAs reminded the licensee to provide care and supervision. ON-SITE FOOD PREPARATION: The facility provides breakfast, lunch and snacks for the children. At the time of inspection, food prep areas were clean and sanitary, and food was properly stored. Filtered drinking water is available to children in care. The LPA observed that the facility\342\200\231s floors, equipment, furniture, and children\342\200\231s bathrooms were clean and in good repair. OUTDOOR INSPECTION: The backyard was enclosed by a fence (wall). The outdoor equipment and toys were in good repair and free of sharp edges. At the time of inspection, the surface of the outdoor activity area was well-maintained and free of any observable hazards. LPA observed three chickens, one white rabbit, and two turtle in the fenced corner of the backyard. The licensee also stated that they have one small dog who is vaccinated in the house. Bodies of Water: During the tour of the backyard LPAs observed a jacuzzi and a fishpond. Licensee has one Jacuzzi in the backyard and a fishpond. The Jacuzzi and a fishpond had a mesh fence that was 5-foot-high and cover that had two locks, one on each of the corners that was locked and secure. The gate of the mesh fence outside the Jacuzzi and a fishpond area has a locked gate that swings away from the jacuzzi and a fishpond with a separate key to the latched gate. Licensee stated that the jacuzzi and a fishpond will not be used and shall remain covered and locked during business hours. Licensee climbed on top of the jacuzzi cover and the cover supported the weight of licensee. LPAs used the bodies of water checklist to meet Title 22 regulations 102417(g)(5). LPAs observed there are one windows providing direct access to the jacuzzi and a fishpond with the mesh fence. LPA reminded Bhorapkar, Anita that she must inspect the backyard daily to ensure that the cover is on the jacuzzi and the fish pond, locked, and secure, the 2 aquariums are safe to children in care, and that all off-limit areas are inaccessible to children in care before operation hours begin. Continued Page 3 Page 3. The garage will not be used for day care. The applicant stated the garage will be off-limits. Or The garage is to be utilized for limited activities; daycare children are not to nap or eat in the Garage. There is a working carbon monoxide, smoke detectors and 2A-10BC fire extinguisher in the home that meet statutory and State Fire Marshall standards. Detergents, cleaning compounds, medicines, and other items that could pose a danger if readily available to children were stored inaccessible to children. There were no poisons or other items observed that could pose a danger to children, and if they were observed, they were locked or inaccessible. Licensee stated there are or are not firearms and or other dangerous weapons in the facility, and none were observed during today's inspections. Licensee stated there is firearms dangerous weapons in the facility. LPA observed it is locked and stored separately from the ammunition. There is a fireplace and/or an open-faced heater in the room #2 screened by a glass cover or metal mesh covering or which is barricaded by a cemented wall, and inaccessible to children in care. The home has a central air and heating system, and the A/C unit is in the side yard, with a fencing barrier across it, making it inaccessible to children. The home has age-appropriate toys for the children it serves. The Licensee is aware that each FCCH shall conduct fire drills and disaster drills at least once every six months. A record review shows that the last drill was documented on 8/8/2025. LPA verified there is a working telephone service (cellular service), licensee was reminded that the childcare phone needs to remain in the childcare at all times. The licensee stated they use the backyard for outdoor play. The licensee does have a current roster of children in care. Zero children records for children present during the LPA inspection time. Children\342\200\231s records for children present during LPA\342\200\231s inspection were reviewed for a copy of the emergency information card that contains all the information specified by regulation (LIC 700), Immunization records, Affidavit Regarding Liability Insurance (LIC282), Consent for Emergency Medical Treatment (LIC627), Notification of Parent\342\200\231s Rights (LIC995A) and found to be in compliance. Licensee stated that there are zero infants under 24 months enrolled in the childcare. LPA reviewed LIC 9227 Individual Infant Sleeping Plan and napping log for infants. The licensee and assistant\342\200\231s Pediatric CPR/First Aid certification expired 6/2027. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states that a person shall not be employed or volunteer at a family childcare home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for (licensee and assistant) were reviewed and within compliance. Continued Page 4. Page 4. Beginning March 31, 2018, H&S Code 1596.8662 requires all licensed providers and employees to complete mandated reporting training and to renew the training every two years. During staff file review, the licensee\342\200\231s mandated reporter certificate expired 5/27/2027 . This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Children are not to be left alone in parked vehicles. When the licensee is temporarily absent from the facility, arrangements must be made for a qualified substitute adult to care and supervise children while absent. The substitute adult must have the required criminal record, child abuse index clearances, immunizations, Pediatric CPR/First Aid, and mandated reporter training. CCLD website www.cdss.ca.gov/inforesources/community-care-licensing was provided to licensee to access regulations, updates, and licensing forms. Licensee was advised to register through childcareadvocatesprogram@dss.ca.gov in order to receive quarterly updates. Licensee was advised of their responsibility to review the Provider Information Notices (PIN) found on the CCLD website. A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the licensee. LPA discussed the safe sleep regulations with licensee Anita Bhorapkar, and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee Anita Bhorapkar of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee Anita Bhorapkar was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. In the areas that were evaluated, NO deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit. Licensee Anita Bhorapkar was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated Exit interview conducted and report was reviewed with the licensee Anita Bhorapkar. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were discussed licensee Anita Bhorapkar, was provided a copy of their appeal rights (LIC 9058), and their signature on this form acknowledges receipt of these rights. During the exit interview, the licensee, Anita Bhorapkar, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. End of the Report.
