Based on observation, interview, and record review, the facility did not comply with the section cited above. During staff file review, LPA observed two out of the three staff were missing mandated reporter certification. Staff was unable to provide current and a mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.
