What Happened
Complaint investigation substantiated that child C1 was left unattended in kitchen for 30 seconds to 1 minute on separate occasions in September 2022. Three other allegations regarding rough handling, yelling, disinfectant access, and meal service were unsubstantiated.
Observations
- Child C1 left alone in kitchen approximately 30 seconds to 1 minute (confirmed by multiple staff disclosures)
- First incident: approximately 1 month before 10/18/2022 investigation, child walked into kitchen unnoticed when staff (S1) went inside
- Second incident: approximately 3 weeks before investigation, similar occurrence
- Staff S1 heard whimpering and found child at kitchen window
- Staff did not ensure children remained supervised in designated areas
- Other allegations (rough handling, yelling, bleach accessibility, meal provision) found unsubstantiated based on staff and parent interviews
