(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. Based on lab report analysis water from outlet C (15.0 parts per billion). Outlet samples were taken on 07/08/2022 indicate levels of lead in exceedance.

This requirement was not met as evidenced by:Based on staff interviews. A staff member disclosed witnessing Staff 2 (S2) raises S2 voice when children do not listen to S2’s directions. It is loud and can hurt the children and staff's ears when S2 raises S2 voice. This staff member also confirmed witnessing S2 yelled at a child while the child was sitting on a chair and said “get up right now” so another child can sit. This poses an immediate risk to the health of the children in care.
