What the inspector wrote

Page 1 of 1 Licensing Program Analyst (LPA), Mahnaz (Nancy) Malek conducted an onsite case management inspection for the purpose of investigating an incident that the facility self reported to our office on 7/7/2025. LPA met with director, Mary Joy Hilsabeck. Census was taken. There was a total of 60 preschool children with 7 staff observed. Eleven children with one staff in PreK, fourteen children with 2 staff in the 2-year-old room, ten children with one staff in Preschool 2, Ten children with one staff in Early Preschool Room, and fifteen children with two staff in TK room. The Village Room and Multi Purpose Room were not in use. According to the self-incident report, on 7/7/2025, the facility representative was notified that Child # 1(C1) disclosed that S1 had taken a picture of C1 possible on the toilet pooping or a snapshot filter of a poop emoji. The date and time were unknown. A review of the Facility Personnel Report Summary on this date indicates all facility who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA interviewed 8 staff, and 8 older preschool children regarding the incident. LPA reviewed and obtained some documents from the facility related to the incident. Additional information is needed to complete the review of the incident. The outcome will be finalized during the next visit, once all relevant details have been obtained. The facility representative, Mary Joy Hilsabeck was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalty of $100.00. The facility representative, Mary Joy Hilsabeck was provided with a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. The exit interview was conducted with the facility representative, Mary Joy Hilsabeck. End of report.
