    Based on reviewing 7 staffs' flies, the licensee did not comply with the section cited above in 7 out of 7 files missing current CPR and 1st aid Certificates which poses a potential health, safety or personal rights risk to persons in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation This requirement was not met as evidenced by a self report incident, and staffs' and child's interviews that S1 left 3 year old child unattended in the hallway during transitioning from one room to another for less than a minute. This is an immediate risk to the health and safety of children in care. This is a repeat same violation within 12 months. An immediate civil penalty of 1000.00 was assessed today.

    101229 Responsibility for Providing Care and Supervision: No child (ren) shall be left without the supervision of a teacher at any time. Supervision shall include visual observation. This requirement is not met as evidence by: Based on self-report UIR and staff interview conducted on 4/12/24, C1 was left alone without any adult supervision. This pose an immediate risk to the health of the children in care. Based on interviews and statements from Director and HR, S3 failed to supervise a child from Pre/K to T/K room. This poses an immediate risk to the health and safety of the children in care..

This requirement is not met as evidence by: Based on LPA’s interview, S1 disclosed witnessing S6 tapping child with the broom on the child’s head and S6 was going to smash the broom over the child’s head and would hurt the child with the broom. The child was crying. This action poses an immediate risk to the health and safety to the child in care.
