What the inspector wrote

An unannounced case management inspection was conducted on 06/02/2026 by Licensing Program Analyst (LPA), Deschampe. LPA arrived at the facility at 9:23AM, in response to a self-reported Unusual Incident Report (UIR) dated 05/19/2026. Present during today’s inspection was the Facility Representative/Director (FR), Leisha Dearing. FR guided LPA on tour of the facility with a census of 26 infants and 8 staff. Total infant enrollment is 29, with current census due to varied schedules. A review of staff criminal records indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearance or exemption and a child abuse index clearance. On 05/19/2026, the UIR was filed with the Orange County Regional Office (OCRO) to self-report an incident from 05/19/2026 at approximately 1:00-1:30PM. The Reporting Party (RP) stated the following, Staff 1 (S1) noticed Child 1 (C1) was refusing the bottle and realized the bottle was labeled with the name of Child 2 (C2). RP stated parents/authorized representatives were not concerned. RP stated staff was re-trained. On this date, LPA made observations, received facility records, and conducted interviews with 2 staff. Due to additional information needed, the UIR needs further investigation . Exit interview conducted and report was reviewed with the facility representative/assistant director, Kara Bumer. A notice of site visit was given and must remain posted for 30 consecutive days. The “Notice of Site Visit” must be posted on, or immediately adjacent to, the interior side of the main door to the facility. Failure to post will result in Civil Penalties of $100.00. End of Report.
