    Buildings and Grounds 101238(a)(1): (a)The childcare center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees, and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met as evidenced by: Based on LPA’s observation, record review and interviews, LPA confirmed the information regarding observing rodents and droppings in the facility. This poses a potential health and safety risk for the children in care.

  • PERSONAL RIGHTS

    (a) The licensee shall ensure that each child is accorded the following personal rights:(1) To be accorded dignity...(2) To be accorded safe...(3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule...This requimrent was not met as evidenced by: Based on interviews with Director and Parent #1, Staff #1 rough handled Child #1 and Child #2 by grabbing and throwing them back on their mat during the facility rest period on 12/27/2023. This poses an immediate personal rights risk to persons in care.

    101427 Infant Care Food Service(g) A supply of bottles and nipples shall be maintained at the infant care center. Bottles and nipples used by one infant shall not be shared with or used by another infant unless sterilized. This requirement was not met: Based on interviews conducted, S1 gave C1 the wrong milk bottle without double checking the name on the bottle. This poses potential Health and Safety risk to children in care.

    101223(a)(3) Personal Rights(a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain……This requirement is not met as evidence by: Based on self-reported incident report and interview with Director, S1 inappropriately handled C1 in a rough manner by gripping C1’s left wrist and pulling them to their nap mat. This poses an immediate risk of personal rights to the children in care.
