Page 1 of 1 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the above program while inspecting the preschool program. LPA met with staff in charge, Gail Reyes who was supervising 8 preschool children on the playground. Melinda Marchese from the sister facility in Costa Mesa arrived during LPA's inspection. LPA observed 10 infants and toddlers with 3 staff in the infant room. A review of staff records on this date indicated that all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions. Based on the interviewed with staff, LPA found out that 3 toddlers belong to Toddler Option Program are commingled with the infant toddler program. Staff stated the facility has combined the children belong to Toddler Option Program with Infant Program due to some staff rsignations. LPA reviewed the children's files and found out Child # 2, 3, and 4 are courently enrolled in Toddler Option Program. LPA obsereved that those children were among the infant toddler program. Staff stated there is one staff for Mondays, Wednesdays, Fridays for Toddler Option Program and they are in the process of hiring another staff for other days to cover the Toddler Option Program. The facility representative, Melinda Melinda Marchese, was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
