INFANT CARE TEACHER QUALIFICATIONS AND DUTIES

Based on observation, interview, and record review, the licensee did not comply with the section cited above in one count out of two counts missing infant/toddler unit for Staff # 2 in the infant room which poses a potential health, and safety risk to persons in care.

INFANT CARE FOOD SERVICE

Based on observation today, the licensee did not comply with the section cited above in 4 counts of out of total 4 counts missing labels either with the name or dates on the baby bottles and food borough from home which poses a potential health, and safety risk to persons in care.

Based on observation today, the licensee did not comply with the section cited above in one count out of one total count of objects having 2 pieces of baby bouncers in the infant room which poses a potential health, and safety risk to persons in care. There were no babies in those equipment during inspection.

Child day care centers serving infants; optional toddler program; The toddler program shall be conducted in areas separate from those used by older or younger children. Plans to alternate use of outdoor play space may be approved to achieve separation. This requirement was not met as evidenced by LPA's observation and reviewing documents C2, C3, and C4 belonged to Toddler Option Program and were commingled with infants and toddlers. This is a potential risk to the health and safety of children in care.

Each employee...influenza vaccination between August 1 and December 1 of each year.THis requirement was not met as evidenced by: Based on records reviewed 1 of 5 staff did not have an immunization card and 2 of 5 staff did not have the flu vaccine or a letter to decline flu vaccine which poses a potential risk to the health and safety of children in care.

1596.8662 (b)(1)On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training ...and shall complete renewal...every two years...training.This requirement was not met as evidenced by: Based on records reviewed 2 of 5 staff were missing certificate of completion for mandated reporter training which poses a potential risk to the health and safety of children in care.
