What the inspector wrote

page 2 of allegation( Classroom has an odor.) Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated (page 2 of allegations; Garbage cans are dirty.There are insects in the facility) In Reference to the allegation that there are insects in the facility; the investigation found; At 11:04am LPA toured Preschool (Room #3) and noted on closed off water faucet, there is a red cup taped onto faucet so as to prevent children from drinking from faucet. There were dead insects on tape. At 11:30am LPA noted live insects in kitchen, in the kitchen sink and where the bananas were stored next to sink. The insects appeared to be flying gnats(small winged insects usually flying in groups). LPA photographed gnats in kitchen sink. Based on LPAs observations the preponderance of evidence standard has been met, therefore the above allegations are found to be substantiated. California Code of Regulations, Title 22, Division 12, and Chapter 1 are being cited on the attached LIC 9099D An exit Interview was conducted. Report was reviewed and discussed. The director was provided a copy of their appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. The Notice of Site Visit was posted. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. LPA informed the licensee of how to access regulations and forms from CCLD websites: www.cdss.ca.gov

This requirement was not met as evidenced by LPA Rivas's observation of preschool classroom at 11:04 am of dead insects on tape covering water faucet and kitchen at 11:30 am observation of gnats on kitchen sink durings todays visit. This poses a potential health & safety risk to children in care.

The requirement was not met as evidenced by LPAs observation at11:50 3 trash cans on 3 play yards were dirty and one had a broken lid, all 3 had some form of decaying waste, banana peel, snack food.This poses a potential health & safetyy risk to children in care
