101238 Buildings and Grounds (a)The child care center shall be clean, safe, sanitary... at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met evidence by: Based on observation and interviews with staff members stating they have observed cockroaches in different areas of the childcare center on different dates. This is a potential Health and Safety risk to the children in care.

101638.1 General Sanitation (e) (e) Washing, cleaning and sanitizing requirements for areas used by staff and children, or for areas that children have access to, shall include: (1) Floors, except those carpeted, shall be vacuumed or swept and mopped with a disinfecting solution at least daily, or more often if necessary. Based on observation and interviews with staff members stating the restrooms and floors are not being cleaned by the janitors, this is a potential Health and Safety risk to the children in care.

REPORTING REQUIREMENTS

101212 Reporting Requirements(d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department...1) Events reported shall include...(C) Any unusual incident ... This requirement was not met as evidence by, records review by CCLD that there was not report for the incident that occurred on 04/1/2022 and the director revealed in a statement that the incident was not reported. This violation can cause a potential harm to the children in care.

Denial, suspension or revocation of license, registration, or special permits; grounds;The department may deny an application for or suspend or revoke any license, registration, or special permit issued under this act upon any of the following grounds and in the manner provided in this act: This requirement was not met as evidenced by;On 02/01/22 & 03/02/22 Director did not ensure the personal rights of persons in care to safe and healthful accommodations and engaged in conduct inimical to the health, welfare, andsafety of persons in care, in that facility staff did not wear face coverings while in the facility, as required by the CA Dept. of Public Health Guidance on the Use of Face Coverings issued 06/18/20 and updated 11/16/20, and an individual mask exception did not apply.

Personal Rights The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidenced by: Based on LPAs observations on 02/01/22 & 03/02/22 for a total of 3 staff not wearing masks and interviews conducted . current mandate by the CA Dept. of Public Health is for all individuals age 2 and up to wear a mask indoors. This poses a potential risk to the health and safety of children in care

Buildings and GroundsThe child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.he licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met as evidenced by LPA Rivas's observation of preschool classroom at 11:04 am of dead insects on tape covering water faucet and kitchen at 11:30 am observation of gnats on kitchen sink durings todays visit. This poses a potential health & safety risk to children in care.

Fixtures, Furniture, Equipment and Supplies Solid waste shall be stored, located and disposed of in a manner that will not transmit communicable diseases or odors, create a nuisance, or provide a breeding place or food source for insects or rodents,All containers used for storage of solid wastes, including moveable bins, shall have tightfitting cover that is kept on; shall be in good repair; & shall be leakproof and rodent-proof. The requirement was not met as evidenced by LPAs observation at11:50 3 trash cans on 3 play yards were dirty and one had a broken lid, all 3 had some form of decaying waste, banana peel, snack food.This poses a potential health & safetyy risk to children in care

Personal Rights 101223(a)(1)The licensee shall ensure that each child is accorded the following personal rights:To be accorded dignity in his/her personal relationships with staff and other persons. This requirement was not met as evidence by C1 being denied by S4 the opportunity to use the restroom, resulting in child urinating on self. This poses a potential health risk to the children in care.
