What the inspector wrote

On 7/28/2025 Licensing Program Analyst (LPA) Karen Navar conducted an unannounced Case Management inspection. LPA met with Director Sylvia Carrillo during the inspection and conducted a tour of the facility. A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Director was operating within the licensed capacity as specified on license. LPA observed 41 preschool children and 5 staff present at time of inspection. On 7/28/2025-LPA Navar observed 1 staff (S1) standing in the bathroom with 1/2 walls with 1 child in the and 14 children in the classroom. LPA identified herself and asked staff their name and how many children do they have in care. S1 stated they have 15 children. LPA counted 15 children in care with S1. LPA then observed 1 staff walk into classroom. 2 (two) staff stated that the children were just dropped off and that they are aware of the ratio. During today’s inspection LPA, Navar discussed with director that staff must be in ratio at all times. LPA Navar provided a copy of Title 22 Regulation 101216.3 Teacher-Child Ratio, and a technical violation was given. Exit interview conducted and report was reviewed with the director Sylvia, Carrillo. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. End of Report.
