What the inspector wrote

On 2/03/2025 Licensing program analyst (LPA) Karen Navar conducted a case management visit due to deficiencies observed during a complaint investigation on 2/03/25. LPA met with Director, Nicole Carreon during the inspection. At 9:25AM, LPA Navar observed 1 staff member (S1) providing care for 5 infants in the Infant classroom. S1 stated they need to call for back up. At 9:30AM Staff 2 (S2) arrived and census was taken 6 Infants and 2 staff. (S1-S2). A review of the Facility Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Based on LPA's observations and interview the facility was operating out of ratio.Therefore, in the areas that were inspected, one Type A deficiency was observed of the California Code of Regulations, Title 22, Division 1 2 101416.5 Staff-Infant Ratio at the time of the visit. See attached LIC809D. LPA Navar informed Director Nicole Carreon that this report dated 02/03/25 documents one Type A citation and will need to be provided to parent/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview was conducted with Director Nicole Carreon. LPA gave a copy of Title 22 Regulation 101416.5 Staff-Infant Ratio to director. The Notice of Site Visit was posted for no less than 30 consecutive days. Appeal Rights were explained.
