On 7/19/2024, Licensing Program Analyst (LPA), V Trinh conducted an on site case management inspection in response to an incident that the facility self reported on 7/15/2024. The director was informed of the visit\342\200\231s purpose and a tour was completed. The census was 66 children, and 5 staffs in the classroom. The facility was operating within teacher child ratios and licensed capacity. An on-site Facility Personnel Report Summary review indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA met with the Center Director, Yareli Juarez. According to the incident report, Child 1 (C1) dislocated their elbow on or about 7/1/24. Facility staff stated that C1s authorized representative stated C1s elbow had a fracture; however, no medical reports were provided. C1 returned to the facility on July 3. According to the incident report a child (C1) dislocated their elbow on or about 7/1/24. During the course of the investigation, LPA interviewed Center Director Yareli Juarez , and 6 staff, obtained a copy of the children's roster (LIC 9040), Employee\342\200\231s handbooks, and C1\342\200\231s record, Family Roster, and Video on the day of incident July 1, 2024. At this time further investigation is needed. Exit interview conducted with Director Yareli Juarez, and notice of site visit must be posted at the entrance of the facility. Notice of site visit is to remain posted for no less than 30 days. Failure to post will result in civil penalties of $100. Appeal Rights was explained. End of the Report.
