Inspector’s narrative

What the inspector wrote

Page 2 During the investigation on 1/25/2023, LPAs interviewed staff, reviewed children files, and received a copy of the children's roster. During the course of the inspection, LPA contacted 10 parents to conduct parent interviews. Six parents from the toddler class were called. Two out of the six parents were able to be interviewed. One parent said they have witnessed their toddler child inside the preschool class and gave specific days their child attends. LPAs conducted a visit on the day the toddler attends and LPA did not observe their toddler or any other toddlers inside the preschool classroom or on the preschool play yard. Parent interviewed, stated never witnessing their child or children playing with older children on the play yard. LPA contacted 4 preschool age parents and was able to interview 3 of the 4 parents. All preschool age parents that were interviewed stated they observed their child or children inside the preschool class and never inside the toddler classroom. LPAs did not observed any preschool age children inside the toddler classroom during the visits. On 1/25/2023 LPAs interviewed five staff. All five staff stated they have never witness staff yelling at one another or at children. While at the facility, LPAs did not observed staff yelling. During parent interviews, all parents were satisfied with the care and supervision their child or children receives at the facility. One parent stated, they have nothing but positive things to say. Based LPA observation on 1/25/2023 and on 2/9/2023, Staff interviews conducted on 1/25/2023, and parent interviews there was not enough evidence to substantiate the allegations that Staff yells at children in care and Staff commingle children of different age groups. Children were unable to be interviewed on 2/9/23. This agency has investigated the complaint alleging Staff yells at children in care and Staff commingle children of different age groups ; although the allegation may have happened or is valid, there is not enough preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Notice of Site Visit was posted. The notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The licensee was provided a copy of their appeal right (LIC 9058 1/16) and their signature on this form acknowledges receipt of these rights. Exit interview was conducted.
