This requirement was not met by: This requirement was not met as evidence by parent interviews conducted on 5/8/2023. Parents stated they have witnessed staff (S1) yelling at children in care. An additional parent stated their child has mentioned to them staff (S1) yells. Children interviews conducted on 5/12/2023. Two out of four children indictated S1 yells at them and their friends. This poses an immediate risk to the safety of the children in care.

When LPA requested S1 file, Director stated she would email the file. LPA observed S1 was not associated to the facility.
