The purpose of this inspection was to conduct an unannounced visit due to a self-reported incident report received on 03/09/2023. Licensing Program Analyst (LPA) Castanon and LPA Gutierrez met with Director Nichole Ontiveros. LPA’s were provided a tour of the inside and outside of two (2) infant rooms (Nido 1 and Nido 2) and three (3) toddler classrooms (Toddler 1, Toddler 2 and Toddler 3) and census was taken. Observed at the time of the visit was a total 28 infant and toddler children with 12 staff in the five (5) different classrooms. A review of the Facility Personnel Report Summary on 03/16/2023 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During today’s inspection, LPA Castanon and LPA Gutierrez interviewed Director and four (4) staff members. LPA’s reviewed eleven (11) child file and one (1) staff file. LPA’s also obtained a copy of the children's roster, personnel report, children’s records, staff training documentation. All documents provided during visit need to be reviewed and further interviews need to be conducted. Due to insufficient information available at this time further investigation is needed. Exit interview was conducted with Director Nichole Ontiveros. The Notice of Site Visit was posted. Director was advised the Notice of Site Visit must be posted for 30 days or $100 Civil Penalty will be assessed. (End of Report)
