This requirement was not met as evidenced by a facility self report incident, interviews and LPA observing a video footage in a classroom that staff # 1 pulled out a Motnessori material out of a 23 months old child's hands inappropriately in an unkind way. This is potential risk to the health and safety and personal rights of children in care.

LPA discovered 11 infant children and 13 toddler age children were missing the LIC 9224 Acknowledgement of Receipt of Licensing Report dated 4/12/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.

101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time.... Supervision shall include visual observation. Based on LPA staff interviews and statements, it was revealed that child (C1) was left on the play yard on 4/10/2023 without any staff supervision. Child (C1) was left unattended on the play yard while staff were inside the facility.

Interview with parent indicated they were not notified about head injury until 2 days later. This poses a potential risk to clients in care
