Inspector’s narrative

What the inspector wrote

Interview with staff: On 10/24/2023, LPA interviewed six (6) staff at the facility. Four (4) of six (6) staff consistently stated child # 1(C1) was isolated at the office when he felt ill. Staff # 1 (S1) and Staff # 3 (S3) stated that when they checked C1’s temperature, they sent it through Brightwheel app to notify parents 2x in the afternoon at 2:45pm and at 4:10pm. S1 stated that S1 contacted C1's parent regarding fever. Interview with children: On 10/24/2023, the LPA interviewed four (4) children at the facility. None of the children disclosed any information that could corroborate the allegations about isolation when they are sick. Interview with Parents: On 11/13/2023, the LPA called eight (8) parents. The LPA reached three (3) parents. The other five (5) parents did not respond to the Department’s request for an interview. All three (3) parents responded similarly stating that the facility overall has good communication and overall satisfied with the care that their children receives. On 11/21/23, LPA Jung tried to get in touch with the Parent #1 (P1) of C1 to get more information about the incident. P1 stated that he received a call from the school notifying him of C1’s temperature. P1 went to the school immediately after the call. Record Review: On 10/24/2023, the LPA received several documents from the facility’s director including sick policy. Based on the documents, a child will be inspected for signs of illness daily and parents will be called immediately if a child gets sick. Based on observation, interviews conducted and record reviews, the complaint alleging (1) Staff do not isolate a sick child upon observation of illness and (2) Staff did not immediately inform child's authorized person upon observation of illness is UNSUBSTANTIATED. Although the allegations may have happened or are valid, there is not enough preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. Exit interview was conducted. Notice of Site Visit was posted during the visit. Director Sun Chung was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights.
