Page 1 of 3 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of the facility to add its toddler option from 6 toddlers to 18 toddlers in the Toddler Option Program. The facility has been licensed for 135 preschool children in rooms 1, 2, 3, 4, 5, 8, and 6 toddlers in Room # 7. LPA met with director, Laura (Gabby) Johnson who gave LPA a tour of the facility. Room 7, and 8 are located on the upper level of one part of the building and rooms 1, 2, 3, 4, 5 are located on the lower level on the other side of the building. All areas identified on the Facility Sketch were inspected. During touring the facility LPA observed 9 toddlers in room # 7 with 2 teachers, and total of 71 preschool children in rooms 2, 3, 4, and 8. Rooms # 1 and 5 were vacant. Room # 8 with 18 napping children and 3 staff, room 2 with 18 napping children with 3 staff, room # 3 with 20 napping children and one staff, and room # 4 with 15 napping children with one staff observed. The facility has requested to provide care and supervision to 135 preschool children ages 2 to 6 years old in rooms 1, 2, 3, 4, and 5 and 18 toddlers ages 18 months to 36 months old in rooms 7 and 8, Monday to Friday from 7:00 am to 6:00 A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA reminded the director, Laura Johnson the infant sleep log for every 15 minutes check on children 24 months old since this program has toddler option component and children under 24 months old shall be checked every 15 minuets during nap time. The director, Laura Johnson was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Continued on page 2 Page 2 of 3 Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. The measurements are as below: Room # 7: Toddler room 217.46 square feet divided by 35= 6.21 (6 Toddlers) Room # 8 Toddler room 570.22 square feet divided by 35 = 16.29 (16 toddlers) Total outdoor space: 519.23 square feet divided by 75= 9.92 (7 toddlers) Facility is planning to add infant outdoor space to the outdoor toddler option yard and request a waiver to share the outdoor space once the increasing toddler capacity is approved. There is one sink in the classroom with a changing table within arm's reach. Room 8 has one sink in the room and 4 toilets and sinks. There is diaper changing table in rooms 7 and 8. Room # 1 is 1058 square feet Room # 2 is 369.92 square feet Room # 3 is 889.07 square feet Room # 4 is 534.69 + 235.64 square feet Room # 5 is 1077.47 square feet The total outdoor preschool rooms after changes is 4165.42 square feet divided by 35 = 119 children. Outdoor space for playground is 4735.62 square feet divided by 75 = 135 children. There are 10 toilets and 10 sinks in the lower level of the building. The director submitted a withdrawal letter to withdraw her application. The facility will go back to the original number on the license for 135 preschool children in rooms 1, 2, 3, 4, 5, and 8 and 6 toddlers in room # 7 The director, Laura Johnson was informed of the MyChildCarePlan.org site, a consumer education website Continued on page 3 Page 3 of 3 that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Exit interview conducted and report was reviewed with the director, Laura Johnson. The following deficiency was cited during this inspection. Section 101161(a) for Limitation on capacity. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. A withdrawal letter and updated LIC 200A was provided to LPA during this inspection. The director submitted a withdrawal letter to withdraw her application for increasing toddler option. The facility will go back to the original number on the license for 135 preschool children in rooms 1, 2, 3, 4, 5, and 8 and 6 toddlers in room # 7 End of report.

    Limitation on capacity- A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement was not met as evidenced by: During taking census at 12:05pm LPA observed 9 toddlers from Toddler Option Component with 2 staff in Room # 7. The license states 6 toddlers in room # 7 based on the measurement taken on 7/1/2021. The facility failed to comply with the section above. This is potential risk to the health and safety of children in care.
