This requirement is not meet by: Based on interviews conducted by LPA Nguyen, it was determined that the facility did not report the child abuse report to licensing. This is a potential Health and Safety risk to the children in care.

    LIMITATIONS ON CAPACITY

    Limitation on capacity- A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement was not met as evidenced by: During taking census at 12:05pm LPA observed 9 toddlers from Toddler Option Component with 2 staff in Room # 7. The license states 6 toddlers in room # 7 based on the measurement taken on 7/1/2021. The facility failed to comply with the section above. This is potential risk to the health and safety of children in care.

  • PERSONAL RIGHTS

    101223(a)(3) Personal Rights. (a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation….. This requirement is not met as evidence by: Based on LPA’s observation on the video recording showing S1 grasp C1 arms while jiggling C1 and documents inisde S1 file.

    Based on observation and interview, the licensee did not comply with the section cited above. While touring the facility at 8:55 am, LPA and Director observed S1 inside classroom #4 caring for 13 preschool age children which poses an immediate health, safety or personal rights risk to persons in care.

  • TEACHER-CHILD RATIO

    Based on observation and interview, the licensee did not comply with the section cited above. At 8:24 am, LPA observed 17 preschool age children inside classroom #1 along with staff #3 (S3) and staff #4 (S4). LPA observed S2 remove two children from classroom #1 and bring them into classroom #5.in [count] out of [total count] [(objects) (persons)] [identifiers] which poses an immediate health, safety or personal rights risk to persons in care.
