What the inspector wrote

Licensing Program Analyst (LPA) P. Rivas conducted an unannounced case management visit and was assisted by Laura "Gabby" Johnson, Director and designated applicant. The licensee is requesting an increase in capacity for infants to 16 ages 6 wks to 24 months in room 6 & 7. Adding room 7 and removing it from the toddler option program under license#304371512. Hours of operation are Monday to Friday from 7:00 am to 6:00 pm. LPA was advised that there have been no changes to indoor and outdoor activity space in terms of square footage and storage. Based report of 07/01/2021 and measurements taken on that date for rooms 6 and 7(under license number 304371512), Total indoor activity space measured at 574.8 square feet , which is sufficient to accommodate the requested capacity of 16 infants . A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Ms. Johnson that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Ms. Johnson stated that they will be in the director office but life saving medication will be kept in the individual classroom, inaccessible to children. . A fully equipped first aid kit is in each classroom. There is an operational carbon monoxide detector on site located , placed in infant room. All required licensing documents were observed posted in the front entrance. Children will be signed in and out in front entrance. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, page 2 chairs, cubbies, cribs, napping cots/mats for infants who can climb out of a crib, bookshelves, and other activity supplies for the children. Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. High chairs and feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. High chairs are made of washable, moisture-resistant material. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from activity space via 4 ft wall. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. Sleep logs are kept in crib area. Needs and Services Plans and Sleep Plans are kept in cabinets. There is no potty training. LPA observed a total of 2 sinks available for children’s use and 2 sinks for diapering. These are sufficient to accommodate the requested capacity of 16 infants. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the director's office and the bathroom will be the adult bathroom within the toddler room. Drinking water is available in the classrooms via own water bottles,. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 06/20/24. The kitchen area currently includes, refrigerator, crock pots, small oven, pressure cookers, The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via locked door. The facility currently has a fully fenced playground area which is separate from other components. Fencing is chain link and is at least four feet high. The total square footage for all the outdoor activity space is 947.99 square feet which is sufficient for 12 infants. page 3 A playground waiver to share the playground at different times in the AM & PM due to limited square footage shall be submitted and approved prior to increase in capacity. Shade is provided via shade sails. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. Drinking water is available via own bottles. LPA observed all hazardous items on the playground to be inaccessible to children. It is noted that the grass area is uneven and is becoming more dirt, some parts are uneven that can potentially be issue with wear, tear, and weather, in the future. A technical violation is issued under 101429 (a) 2(C)3Responsibility for Providing Care and Supervision for Infants ; Documentation shall be maintained in the infant’s file and be available to the Department for review. Documentation shall include the following:Time of For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). page 4 The following corrections are needed prior to the issuance of the license: 1. An approved waiver request for infant yard. 2. Make sprinkler piping and control valve inaccessible to children in care. 3. Final review. Applicant/Licensee Representative Ms. Johnson understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted, and report was reviewed with the Director Laura (Gabby) Johnson.
