What the inspector wrote

On 05/09/2022, during the initial 10 days of inspection, LPA Tina Nguyen observed 7 staff members provided care for 32 preschool children. LPA Nguyen interviewed 7 staff members. All interviewed staff denied of witnessing any staff yelling at the children. Interviewed staff members indicated that they never witness any staff member was on the phone and not pay attention when the child fall. All staff members stated if the children fall, they will check on the child injure area, clean or apply ice-pad if need, complete the ouch report to tell the parents. During the inspection on 05/09/2022, LPA Nguyen interviewed four children. All interviewed children like their teachers. No children disclosed witnessing any staff talking on the phone in the classroom or outside in the playground. The children also stated if the children fall, the teacher would come them make sure they are ok, tell them to be careful and tell the parents. LPA Nguyen contacted six parents for interview, was able to talk to three parents. Interviewed parents did not have concern with the teachers and the facility. Based on the information gathered from LPA' interviews, observation, and reviewing records, there is insufficient evidence to corroborate the allegations of Staff screams at day care children; Staff are not adequately supervising day care children. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged Staff screams at day care children; Staff are not adequately supervising day care children did or did not occur, therefore the allegations are UNSUBSTANTIATED. An exit interview was conducted. The Notice of Site Visit was posted. The facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in a civil penalty of $100. “The licensee was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights.” This report LIC 9099 was provided to the licensee. First level appeal is to Regional Manager, the address is above on the report.
