Pre-licensing visit·June 15, 2022

Applicant Ms. Leslie Lucy Licensing Program Analyst (LPA) Ms. Ketki Desai conducted an announced on site pre-licensing inspection at the childcare center. It is a New center. LPA met with the Owner/ Applicant Ms. Leslie Lucy who gave a tour of the Child Care Center on the premises of The Crossing Church. Three rooms and outdoor play yard is designated to the Child Care Center. The applicant has requested to provide care and supervision for 60 Pre-school age children 2 to 6 years of age, Monday through Friday 7:30 AM. to 6.00 PM. in the assigned three classrooms. (Room # 101, 102 and 103) Today's inspection the play structure on the Preschool yard was reinspected. The Original manufacturer label was for ages 5 and above not recommended for younger preschool age children. The structure now has been modified and a new manufacture label has been posted which indicates it is appropriate for 2-12 ages of children. LPA observed the height has been reduced and is completely safe for young preschool age children to play. The structure is placed on the a soft green turf serving as a cushion to absorb the fall. Based on today's inspection the new play structure and preschool yard are appropriate for the preschool age children 2-6 years in the facility. A license for 60 Preschool age children (2-6 years) in three assigned classrooms # 101/ 102 and 103. Monday to Friday 7.30 am to 6.00 PM, shall be issued upon management approval. Report read and appeal rights presented to the applicant.
