Inspector’s narrative

What the inspector wrote

(Page 1) Licensing Program Analyst (LPA) Giselle Lucero made a follow-up proof of correction visit in response to 1 Type A violation that was issued on 08/19/2025. LPA was met by Parent Liason, Jenna Tashima, who guided the analyst on a walk through of the facility. LPA observed 5 children with 1 staff in classroom 101, 5 children with 1 staff in classroom 102, and 8 children with 1 staff in classroom 103. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The deficiency that was cited on a LIC 809 dated 08/19/2025 has been cleared. LPA observed the in-ground baptism pool and water falls/fountains above the pool did not have water and were completely emptied. LPA observed pool safety net still installed. During the visit, Jenna Tashima and Amber Martin shared that the church's use of the baptismal pool creates a lengthy preparation process each time the pool is needed for baptisms. To address this, they are considering purchasing a pool cover to reduce the time and resources spent on repeated preparations. LPA provided them with PIN 25-01-CCP to ensure they are aware of the specific type of cover required to remain in compliance with licensing regulations and to inform the department once changes are made. POC letter given, and correction has been received at this time. Exit interview conducted and report was reviewed with Parent Liason Jenna Tashima and Director Savannah Davis. A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided. End of report.
