What the inspector wrote

Based on the corroborating information collected LPA Lea Habtom has found that the allegation licensee did not provide licensing reports to authorized representatives to be SUBSTANTIATED: meaning that the allegation is valid because the preponderance of the evidence standard has been met. Title 22 deficiency was cited on 9099-D. This report was reviewed with licensee, Rosa Pena De Licata. A notice of site visit was provided to be posted for 30 days. Appeal rights provided.

    Criminal Record Clearance 102370(d)(1): All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing, or volunteering in a licensed facility:(1)Obtain a California clearance or a criminal record exemption as required by the Department this requirement was not met as evidenced by a paid adult providing assistance to children with no fingerprint clearance which posses an immediate health and safety risk to the children in care.
