What the inspector wrote

On August 15, 2025, Licensing Program Analyst (LPA) Lea Habtom, arrived at the facility to close a complaint. There were 5 preschool children being supervised by the director. One more child arrived during the inspection bringing the census to 6 children. All staff present today have fingerprint clearances and associations. During the inspection, a case management was created to address a lack of supervision. The director was in the bathroom changing a child's pull up and another child was washing their hands. There were 3 other children who ran to the back room playing on a table with no staff supervision. The room is located to the back left of the main room that is used for napping. When the director finished changing the child's pull up, the director escorted the children back in to the play room where the bathroom is and shut the doors. Around 12:00 pm, another teacher arrived and went upstairs to the infant room. The aide in the infant room then came downstairs to be with the director. LPA Lea Habtom informed the director Patricia Perez that this report dated August 15, 2025, documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Lea Habtom informed the director Patricia Perez to provide a copy of this licensing report dated August 15, 2025, that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

    101229(a)(1) Responsibility for Providing Care & Supervision: The licensee shall provide care and supervision as necessary to meet the children's needs. (1)No child(ren) shall be left without the supervision of a teacher at any time. This requirement was not met as evidenced by: Three children were in the back left room with no staff supervision playing as the director was changing a child's pull up in the bathroom which is an immediate health and safety risk to the children in care.
