This requirement was not met as evidenced by the facility who did not contact 911 when a child had a medical emergency and the appearance and responsiveness of the child changed which is an immediate health and safety threat to the children in care.

This requirement was not met as evidenced by: Three children were in the back left room with no staff supervision playing as the director was changing a child's pull up in the bathroom which is an immediate health and safety risk to the children in care.
