What the inspector wrote

On Thursday, September 11, 2025, Licensing Program Analysts (LPAs) Lea Habtom and Matt Gallo arrived at the facility for an unannounced inspection to deliver the findings for the complaint filed on July 3, 2025. At the time of LPAs arrival, at 11:40 AM, there were 2 staff, S1 & S2, supervising 5 preschool children. LPAs met with director, Patricia Perez, who was upstairs in the infant room supervising 4 infants. The two staff downstairs do not meet the requirement of a teacher or a fully qualified teacher and are currently considered an aide. Staff #1 has 9 units and is missing the Child, Family and community course. S#1 is not registered for the course. Staff #2 has 3 units in child development and is not enrolled in any other courses. Based on today's inspection, a Type A citation was issued due to staff qualifications. This report was reviewed with director, Patricia Perez. A notice of site visit was provided to be posted for 30 days. Appeal rights provided. LPA Lea Habtom informed the director Patricia Perez that this report dated September 11, 2025, documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Lea Habtom informed the director Patricia Perez to provide a copy of this licensing report dated September 11, 2025, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
