What the inspector wrote

On Tuesday, July 1, 2025, Licensing Program Analyst (LPA) Lea Habtom arrived at Guiding Stars for an unannounced case management inspection. LPA met with director Amber Willams and owner Mallory Shepard. There were 21 preschool children outside being supervised by 2 staff and the director. All staff present today have fingerprint clearances and associations. The purpose of today's inspection is in response to the self reported incident on June 24, 2025. LPA interviewed director, Amber Williams, regarding an incident on June 23, 2025, around 4:30 pm, where a child was inadvertently left outside in the gated play area with no supervision during transition from outdoor free play. A parent notified staff after seeing the child outside and assisted with bringing the child back inside. Based on the information collected, the facility was issued a Type A and a civil penalty. This report was reviewed with director, Amber Williams. A notice of site visit was provided to be posted for 30 days. Appeal rights provided. LPA Lea Habtom informed licensee Amber Williams that this report dated July 1, 2025, documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Lea Habtom informed the licensee Amber Williams to provide a copy of this licensing report dated July 1, 20205, that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

This requirement was not met as evidenced by: an incident report and the interview with the director that disclosed a child was inadvertently left outside with no supervision which is an immediate health and safety risk to the children in care.
