On Tuesday, September 2, 2025, Licensing Program Manager (LPM) Mai Lor and Licensing Program Analyst (LPA) Lea Habtom, met with licensee, Mallory Shepard, for an informal office meeting. LPM Lor defined the difference between a non-compliance conference and an informal meeting. LPM advised that the purpose of today’s meeting is to help the facility staff gain compliance. Today’s informal meeting is to discuss Type A citations issued on 7/01/2025 for absence of supervision and 7/23/2025 for not adhering to the limitations of the capacity on the license. On 07/1/2025, the facility was cited a Type A citation for a child who was left outside during transition from outdoor play time back into the facility. On 07/23/2025, the facility was cited a Type A citation for commingling infants under 2 years old with preschool children over 2 years old. The facility representative stated that they have taken the following steps to maintain compliance: -A staff meeting was conducted to reinforce the name to face and head count -A second teacher will come out to assist during transition time from outdoor play to inside to conduct the name to face and head count policy -Reinforce with teachers that when the chilren come inside to do name to face and head count LPA and LPM reviewed the supervision and capacity regulations. LPA provided the technical support program to the facility representative. This report was reviewed with all staff present during today's meeting. A copy of the report was provided to licensee, Mallory Shepard.
