What the inspector wrote

LPA informed Facility Representative (FR) Amber Williams, that this report dated 02/26/26 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA informed the FR to provide a copy of this licensing report dated 02/26/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative (FR) Amber Williams. Appeal rights provided.

    This report has been amended from the original report dated 02/26/26.101223 Personal Rights(a) The licensee shall ensure that each child is accorded the following personal rights:(3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to:...This requirement is not met as evidenced by: Based on witness statements and documentation reviewed, it was determined that a staff yelled at day care child after believing the child was attempting to run out the door. It was further confirmed that the staff member's tone was inappropriate and that yelling at close proximity to the child’s face was unnecessary which poses an immediate health, safety, or personal rights risk to persons in care.
