(1)No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by the facility self-reporting that on 03/10/2026, Child 1 eloped from the facility temporarily without supervision from staff before being retrieved by facility representative Navneet Baring.

This requirement was not met as evidenced by admission. Facility representative and staff stated that on 10/25/2024, C1 was left unatttended in a classroom for an approximate time of no more than a minute. The child was observed by administration shortly after the classroom had transitioned outside. This was considered an immediate risk to the health and safety of chidlren in care.
