What the inspector wrote

At 4:25pm, Licensing Program Analyst (LPA) Matthew Gallo arrived at the facility for a collateral visit. The purpose of the visit was to interview a staff member regarding a complaint investigation for the infant license 313624805 at 801 Sterling Parkway. At 4:15pm, LPA had met director Paolo Sarmiento at that location. Upon arrival at this facility, LPA observed 16 school aged children supervised by 2 staff. During the visit, LPA conducted staff interviews. During the visit, LPA observed deficiencies. Upon arrival at 4:25pm, they observed 16 school aged children supervised by 2 staff. Minutes after LPA's arrival, one of the staff members (S1) left, leaving the 16 children with the second staff member (S2). Through interview and record review, LPA determined that S2 did not the have the necessary college units or training hours to qualify as a teacher. Therefore, the 16 children were left under the supervision of an aide and remained under the supervision of an aide when LPA left at approximately 4:35pm. A Type A Title 22 Deficiency is cited on the accompanying 809-D Licensee acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided. Exit interview conducted and report was reviewed with the facility representative, Paolo Sarmiento. A notice of site visit was given and must remain posted for 30 days.
