What the inspector wrote

At 9:20am on 2/26/2026, Licensing Program Analyst (LPA) Matthew Gallo met with facility representative Paolo Sarmiento to conduct a plan of correction visit. During the visit, LPA observed a total census of 6 school age children supervised by 1 staff. The licensee was previously cited a Type A deficiency on 2/13/2026 due to transporting children in vehicles that did not have valid registration or legally required liability insurance that would cover injuries to passengers in the car. LPA verified that insurance had been renewed on 2/13/2026, and the plan of correction stated that the licensee will not transport in either vehicle until they provide LPA with proof of valid registrations. During today's visit, LPA observed valid registration for a 2005 Dodge Caravan, and determined through staff interview that this is the car currently used for transportation. The plan of correction has been fulfilled and is therefore cleared. Exit interview conducted and report was reviewed with the facility representative, Paolo Sarmiento. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights.

This poses a potential health, safety, or personal rights risk to persons in care.

    101299(a) The licensee shall provide care and supervision as necessary to meet the children's needs.This requirement was not met as evidenced by: Based on interview, the licensee did not comply with the section cited above due to staff not providing supervision adequate to notice that nail polish was being used by children in care, leading to a child sustaining nail polish on their scalp. This poses an immediate health, safety, or personal rights risk to persons in care.
